最近通い始めた歯医者さんで上の第一小臼歯の治療をすることになりました。

最近通い始めた歯医者さんで上の第一小臼歯の治療をすることになりました。この歯は銀歯の詰め物をしているのですが、中が虫歯になっているそうです。
治療後は白い詰め物を希望しているのですが、第一小臼歯は保険適用で白い詰め物ができるのでしょうか。
今通っている歯医者さんでは、保険適用の白い詰め物は使われていないそうで、白い詰め物をする場合は、保険適用外になり4万円前後するそうです。見た目や強度の問題もあるかもしれませんが、費用の面で保険適用で治療して頂きたいと思っています。

ご相談の件ですが…詰め物や被せ物という呼び方は、歯医者さんが患者さんに分かりやすく説明する時の呼び名で、実は歯科医療における正式名称ではありません。
なので、かかられてる歯医者さんによって、どこまでを詰め物と呼ぶか被せ物と呼ぶかは、微妙に違ったりするのですが…当院では、
①溝の部分のみの限定的な虫歯〜溝から歯と歯の間にまで伸びているが、歯の表裏のコブを超えていないもの。
または、
②歯と歯茎の境目の虫歯に行う治療
…を、詰め物と呼んでいます。今回は、その定義で回答させていただきます。
さて、ご相談の詰め物は、ニュアンスから①と考えます。
その場合…前者、歯の溝のみの虫歯の場合なら、原則的に保険で白い詰め物は可能と思われます。
後者…歯と歯の間まで及んだ虫歯の場合は、治療を行う歯科医院の信念によって、取り組み方が異なります。当院では、型取りしての詰め物をお勧めしており、保険なら金属になります。
詰め物には、お口の中で直接詰めていくものと、型取りして模型上で作ったものを患部につけていくものがあります。
溝の部分限定の虫歯治療には、直接詰めていく方法がよく用いられます。
溝限定の場合、詰め物は歯という人体最強の額縁に守られ、また比較的単純かつ正確に詰められますので、保険治療のメリットである安価な費用で、精度も耐久性もある程度信頼できる白い詰め物による治療が行えます。
歯と歯の間に及ぶ虫歯の治療では、精度と耐久性のハードルが一気に上がります。
そういったケースにおいて、直接白い歯科材料を詰める方法では、歯の複雑な三次曲面を正確に再現するのは至難の技です。それが施されているケースもありますが、その多くは不正確なものです。当院では口腔内カメラを用い、顕微鏡レベルまで術野を拡大して精密な治療を行っていますが、このレベルで歯と歯の間まで及んだ虫歯…特に臼歯部…の直接詰める方法で高精度なものは、あまりお目にかかりません。肉眼でチラッと見る限りではきれいに見えても、段差があったり、隣の歯との触れ加減が不正確であったりするものがほとんどです。それでは、治療した時は「白くて早くて安い」と喜んでいただけても、虫歯の再発の原因になりかねません。
一方、型取りしての詰め物は、お口の外で作り、なおかつ対象歯だけを取り出して作成作業も行えますので(当院が依頼している技工士さんも、作成の際に専用の拡大鏡を使用してくれています)、高精度な治療が可能になります。
実は、この型取りしての詰め物にも、保険でできる白い詰め物も…あることは、あります。ですが、そう言った治療を行なっている歯科医院は、ほとんどないと思います。強度に致命的な問題が有るからです。
保険の白い詰め物と言うのは、樹脂…プラスチックのようなもの…で、できています。先に申し上げた人体最強の額縁に守られていれば大丈夫でも、歯と歯の間まで拡大した虫歯の治療では、お隣の歯と接触する部分でL字型に曲がる部分ができますね。この部分で折れてしまうリスクが高いのです。なので、保険では強度と耐久性、また特性上精度にも優れる金属の方を選択する歯医者さんが多いのが現状です。
長期的な治療の安定と、見た目の白さ・美しさの両方が求められる場合に選択されるのが、いわゆる自費診療です。セラミックやそれに近い特殊素材等を用い、高レベル・高バランスの治療を実現します。
歯医者同士の話で、「白い、早い、安い」で全部高品質な治療できれば、患者さんも嬉しいだろうね…なんて言うことがありますが、現実的にはそれが難しいことも多々あります。
いまかかられている歯医者さんは、メールで拝見したご相談内容からは、治療内容・費用面で、誠実なご提案をしてくださっていると思います。
お気持ちはよくわかりますが…医療に限らず、より多くのメリットを享受するには、通常ある程度のコストも生じることは多い…かも知れません。
いま悩まれている小臼歯に、保険で白い詰め物をしてくれるという歯科医院も、探せばあるとは思われますが…もしその相談をされる際には、この回答内容をよくお読みいただき、メリットデメリットを、担当の先生に納得いくまで聞かれるようお勧めいたします。
