ご相談の件ですが…まず接着ブリッジにつきましては、当院では原則おすすめしておりません。
まず基本的に、通常のブリッジが「削って、被せると同時に接着もする」ので「接着力」と共に強力に「嵌合力(蓋が嵌まり込む力)」も発揮して外れにくく作っているのに比べ、接着ブリッジはその名の通り、基本的に接着力だけで着けているため、極めて脱離しやすい(外れやすい)からです。
いわば「突っ張り棒」のような構造で「横＝水平」方向に糊付けで付けただけですので、その部位に「上＝垂直(歯科医学的には上顎か下顎かに関わらず、「歯の先が上、根が下」と考えます)」から「噛む」という大きな力が加われば、歯を削ってしっかりと入れるブリッジに比べればはるかに容易に外れてしまいます(いわば「剥がれる」というのに近い現象です)。
加えて、歯と骨の間には「歯根膜」という、(関節で言えば軟骨にあたる)コラーゲンでできたクッションがあります(歯の根を包み込むような形に存在しています)。この厚みの範囲で(目視では確認できない範囲ですが)歯は動いています。加えて、骨にもたわみ•弾力があり、全くの不動ではありません。よって、突っ張り棒を付けている両壁が常に動いていることになり、これも接着ブリッジを外そうとする応力(突っ張っている壁側がたわんで「緩む」)となり得ます。
さらに言えば、…ご相談の部位は「前歯」とのことですので…臼歯＝奥歯なら比較的直線的に並んでいることが多いですが、前歯には通常アーチがあります。「両方向の角度が違う壁」「たわむ方向も異なる」この条件のところに接着力だけで咬合(噛み合わせる)力や咀嚼(食べ物を切るorすり潰す)に構造物が耐えられるかと問われれば、答えは自ずと導き出されます。
それではと…少しでも外れにくくしようと、補助的に少しだけ削って「嵌め込み」を作ると…はめ込みの表面積が少ないため、そこに噛み合わせで生じる応力(常にこじられている)が集中し、嵌め込み部分周囲の歯面がかけること(歯の破折)も懸念されます。そのせいで嵌め込みが緩くなれば、やはり外れます。またその際、予期せぬ方向にささくれ立つようにかけた場合、かけ方にもよりますが、ブリッジの橋桁になってくれている歯にまでダメージを与える可能性も否定できません。
確かに接着ブリッジには「歯を削らずに済む」「他の治療に比して、安価に済むケースもある」というメリットがあります。
しかし上記のように…裏を返せばそれ以外に、あまりメリットがないとも言えるかと思います。
この解説をご理解いただいた後に、ホームページやSNSで接着ブリッジを紹介されている歯科医院さんの説明を読んでいただくと、
「あ、なるほど‼︎」
「そういうことだったのか‼︎」
と納得いただけるかと思います。
これを踏まえた上で…まずもちろん全身状態の確認に始まり、お口全体としてどういう状態か、局所的に問題の部位がどうなっているか、接着ブリッジやスタンダードなブリッジを行うとしてその支台歯(橋桁になってもらう歯)の状態、周囲の歯茎や骨の状態、それらを全て把握した上で、問題の解決方法にどんな選択肢があるか、それぞれのメリットデメリットをご提案•ご相談の上治療を実行するという流れになります。
その上で、適応が可能で、納得いただき、他の治療の選択肢のメリットデメリットもしっかりとご理解いただいた上でそれでも「接着ブリッジがいい」とおっしゃるのであれば施術は可能ですが…もし自分の家族や親友に頼まれても、私ならやりません(価値観や信念の問題ですので、色んなご意見があると思います。他の歯科医院さんの方針を否定する意図は全くありません)。
一方、おっしゃられるご費用であれば、他の選択肢(スタンダードなブリッジ、部分入れ歯、インプラント等)を熟慮された上でお決めいただいても遅くはないと思います。
